Nursing Home Transition (NHT) provides people with the help they need to move back to the community and then provides services to them so that they can live comfortably and safely.
NHT is part of a national trend to serve people in the community rather than in nursing homes.
How does NHT work?
NHT staff will visit nursing homes to identify those people who want to move to the community. While you are in the nursing home, if you choose to move back to the community and you are eligible for NHT, you may be assisted in:
Finding housing
Making your home accessible
Planning your move
Arranging for personal assistance (attendant care) services and other services in the community
In the Community
The NHT team will provide service coordination, personal assistance (attendant care) services, and help with transportation, training and other services based upon your needs.
Who is eligible?
Anyone who is 18 years of age or older who is residing in a nursing home in Pennsylvania and is interested in returning to the community is eligible for NHT.
